MEDICAL SOCIAL WORKER

Our Medical Social Workers are trained professionals who work in a number of similar environments. These could be assisted care facilities, hospitals, convalescent homes or others. Part of the job of these social workers is to help assess individuals for discharge or to see to their needs or the needs of their family during hospitalization.

  • Cooperate with other professionals to evaluate patients' medical or physical condition and to assess client needs

  • Advocate for clients or patients to resolve crises

  • Collaborate with other professionals to assess client needs or plan treatments

  • Interview clients to gather information about their backgrounds, needs, or progress

  • Intervene in crisis situations to assist clients

  • Refer clients to community or social service programs

  • Refer individuals to educational or work programs
